Most men lose their hair for genetic reasons from a condition known as androgenic alopecia or male-pattern baldness (MPB). DHT is a by-product of testosterone. Testosterone converts to DHT with the help of an enzyme stored in the hair follicle’s oil glands. The enzyme is known as 5-alpha reductase. Once hair follicles sense the presence of DHT, they shrink and shorten the lifespan of each hair. Eventually, the follicles will stop producing hair altogether.

Finasteride
Finasteride was developed in the 1970s and licensed by FDA in the 1990s, first as Proscar, a treatment for benign prostatic hyperplasia (BPH, or enlarged prostate gland), and later as a treatment for male hair loss under the brand name Propecia.
Since then, a generic version of finasteride with the same efficacy has been sold on telemedicine platforms in various forms, such as tablets, solutions, sprays, and so on, and is commonly combined with additional active ingredients such as minoxidil.

Dutasteride
Dutasteride was patented in 1996 and received FDA approval in 2001 as Avodart, a benign prostatic hyperplasia (BPH) treatment. Since it has not yet been approved by the FDA specifically for hair loss, it can only be prescribed “off-label” in the US, but this does not mean it’s ineffective in preventing male pattern baldness – quite the contrary.

Inhibiting 5-alpha reductase: Despite their differences, both medications act in the same way to prevent testosterone from being converted into DHT by inhibiting 5-alpha reductase (5AR). Finasteride inhibits type 2, whereas dutasteride inhibits both type 1 and type 2 of the enzyme, preventing more testosterone from being converted.
Half-life (how long it stays in your body): Finasteride and dutasteride differ significantly in their half-life. Dutasteride has a longer half-life (~5 weeks), meaning a single dose can remain in a man’s body for over a month. By comparison, finasteride only has a half-life of approximately five to six hours – it simply has less time than dutasteride to deliver the results you want.
However, the coin is flipped when we start looking at topical applications. Finasteride molecules are smaller than dutasteride molecules, allowing them to penetrate deeper and more efficiently into the scalp.
